Once your child is enrolled, use the links below to find all back-to-school paperwork before the first day of attendance!

• Enrollment Contract (coordinate with Admissions Office)
• Student Information Form & Permissions
  Every year you must complete signed releases for your children. Please follow the steps below:

1. 1. Log in to BigSIS at https://bvwaldorf.bigsis.com/portal/
   2. Click here for help retrieving your username and password.
   3. Go to“Parent/Guardian” tab across the top.
   4. Click on “Requests for Information” tab along the left.
   5. Click on “Fill out Form” to complete the questions to update your emergency contacts as well as permissions for this school year. Whichever guardian starts process first will need to “claim form” and complete information on behalf of your child.

• Immunization Electronic Certificate
  The completed certificate Immunization Record is due whenever your child receives new vaccinations, such as “kindergarten immunizations” or their tDap vaccine. If you refuse any vaccines for your child, you still need to complete the online module and submit a Non-Medical Exemption. You can either complete the online module to sign an electronic copy of the exemption or have one signed by your doctor. The Non-Medical Exemption is due each year.
• Health Appraisal required for all children in Pre-K and Kindergarten AND all children with special health requirements or emergency medication, and all newly enrolling students 
  Health Appraisals expire 1-year from the date your child was last seen by a doctor for a health appraisal. Health appraisals must be completed by a physician each year.
